Notes:	This class is for Indiana University Dwyer School of Health Sciences students, faculty and staff. Class will be held in the Parkside building in the Assessment Lab on the second floor. Please bring cash for payment of the $25.00 certification cards will not be available until fee is paid. If the door is locked call 317-319-0865.

Community Basic Life Support (CPR) course fees are determined by the individual Instructor.  Course fees are to be paid directly to the Instructor.  Instructors are responsible to pay all related material fees to Training Center unless prior arrangements are made.  Please be aware that eCards will not be sent until the responsible Instructor has paid for the eCards and all related fees.
